Claim shall be submitted prescribed formats (Attachment 1 & 2 of the agreement)

Annexure-I of agreement duly by representation of IP duly authorized by a board resolution of the IP. |

3. Claim are accompanied by a pre-receipt with revenue stamp and the claim is accompanied by CD Rom in Ms. Excel format besides hard copy. |

Claim shall be prepared in prescribed give in Annexure-2 of the agreement along with Attachment 1 & 2 showing the computation of payment. |

A self certificate giving details of mobile services sites set up shall be submitted in Performa given in Annexure-9 of the agreement. |

Payment statement shall be certified as per Annexure-I by a representation of the USP duly authorized by board resolution of USP. |

Besides hard copy, USP shall submit the payment statement in CD Rom (CD Rom Disc duly signed by authorized signatory of the company). |
